Providers: Provider Contracting

Provider Contracting

When it comes to patient care, Avesis sets the standard for excellence. Our Quality Assurance Program ensures that all members receive the best vision care available, and that starts with our providers.

Recruitment

Avesis recruits and retains high-quality healthcare providers based on provider interest and patient recommendations. Often, we will reach out to our members’ choices, but it’s easy—and beneficial—for you to start the process yourself.

How to Apply

To get started, send us a message, and one of our recruiters will get in touch with you shortly. You’ll need a few things for the application and credentialing process:

  • Education
  • Professional Background
  • Licensure (current)
  • Diagnostic Pharmaceutical Agent Certification and/or Therapeutic Pharmaceutical Agent
  • Drug Enforcement Agency license (if applicable)
  • Diagnostic Pharmaceutical Agent Certification and/or Therapeutic Pharmaceutical Agent Certification, whichever is applicable

In order to meet our credentialing requirements, you must have a valid license with no pending quality of care complaints. This is verified with State Boards and through the National Practitioner Data Bank. Download an NPI Application to begin the NPI number application process.

See our Credentialing Process for more complete details on requirements.
